'Phonogram Ltd. London' -- and 'Manufactured and marketed by PolyGram
Records'). Number on disc is 314-526-091-2.

Available in most record/CD shops like Media Play, Camelot, etc.

They will file it ~anywhere~ I found mine in the Movie Soundtrack section.
